Ordering Full Sequencing

1. Download and fill out this order form (using macro's on in excell). Print the form after completion using the print button on the 'reactions' page.

2. Prepare your samples as indicated below:

Raw PCR Product Purified DNA
Bacteria
Please use our special vials, except for bacteria.
3. Number the DNA samples by using the white labels and the primers by using the red labels.

4. Send (in padded envelope) to BaseClear:
  • your samples in a BaseBox (please do not use a BaseBox for sending bacteria)
  • filled out order form
